i think at this point Diana is probing Jacks mind to discern what is going on inside of him. This is her walking through the corridors of his mind to find the red door, past the red tendrils, and the origin of the voice.
As much as I can discern from her facial expression, and the hazy corridors in the background, seems to support this idea. It also just makes a lot of sense. Who else is going to get inside his mind and figure out what's going on?
Unusual for what is normal for in-universe. It's not rocket science.
I can only remember her ever doing this with other empathic or psychic characters.

But it’s not *more than capable*. It’s *capable only with telepaths of her mother’s species and one other person with whom she shares a remarkable emotional bond*. This is hardly “Spock telepathically communing with a giant antimatter cloud creature or sensing V’Ger approach from light years away or transferring a copy of his mind at a touch” territory. It’s a remarkably rudimentary implementation of telepathy even given what we see elsewhere in *Star Trek*.
Empathy and Telepaths that master their crafts, are indistinguishable from one another. Look at T’Rina and Book, Michael and Sarak. That mind meld is exactly what it feels like when two people traveling in astral collide into one another’s minds. I’m sure I’m going to here shit about referencing Disco, but I stand behind it! I love that fucking Mushroom Drive! It’s definitely a study on GenZ, and I heard them through Discovery. It had good moments.

Theory: Certain doom but we're looking at Deanna but it's actually Riker.
A lot of folks were concerned because in that shot in the trailer there is a flash of light in this scene. We've been told that "no one is safe" in this season and I've seen some folks postulate that Troi is about to get blown up.
I think that would be a cruel fate. Will is finally progressing in his grief for Thaddeus and to add Deanna's loss to that, plus his now having to raise Kestra alone is too much for our guy Number One.
What if Riker has to sacrifice himself and he's the one in that hallway? By this point, Troi has joined everyone on the ship and she links with Will to be with him in his final moments.
In episode 4 we learn that Deanna hasn't felt anything from Will in a long time. We have his story arc of getting back in touch with himself and his sense of wonder at the cosmos. He is processing his grief and digging into deep emotions about family. We see this in his conversations with Picard about Jack. I suspect we'll see more as he reunites with more of his Enterprise family.
His arc could arguably be coming to an endpoint. He even got to captain the Titan again. I can see a situation where someone has to give it all and Riker goes. Deanna is with him telepathically at the end. She is able to feel his grief, loss, and regret but also gratitude, love for her and the kids, and pride in his duty and career.
I'm just spitballing. I hope it isn't so. But if we lose one of the OG TNG (besides, you know, Ro) I think Riker makes the most sense.
